For a copy of your medical records:

1. Please click the link below to submit a medical records request; Do NOT submit your request in person. 
2. There is no charge for your medical records.
3. If you need imaging (e.g. MRI, X-Ray, CT Scan) in addition to medical records, there is a $6.50 charge for digital images. Please use the same link below.
4. You should receive your records within 5 business days of completing your request.
